I. APPLICATION FORM: (Original and 6 copies)

Must be completed online or printed out and filled in on a typewriter. HANDWRITTEN APPLICATIONS ARE NOT ACCEPTABLE!!!!
ORIGINAL AND SIX (6) COPIES OF COMPLETED APPLICATION FORM MUST BE SIGNED AND DATED.
II. ENCLOSURES WITH APPLICATION (non-returnable):
Seven (7) photocopies of expense page(s) from current graduate catalog ATTACHED to back of EACH application copy
One (1) photocopy of current driver's license, state issued ID card, passport or voter registration
III. DOCUMENTS TO BE SENT UNDER SEPARATE COVER (non-returnable):
Three (3) completed Scholarship Recommendation Forms are required with Letter of Recommendation attached. This is crucial, due to the fact that we look for very detailed info and various opinions on all scholarships. If a Recommendation Form and a Letter of Recommendation is not attached, your application is not complete. Forms are available online. Two (2) of the Recommendation Forms must be Professional Recommendations from educators, professors, employers, or individuals who know the applicant and are in the field of work or studies that the applicant is pursuing. One (1) Scholarship Recommendation Form must be from an individual (non-relative) who has known applicant long enough to comment on character, abilities, motivation, background, etc. of applicant. Recommendation Forms must be mailed directly from individual making the recommendation to National Association of Junior Auxiliaries, Inc.
Certified transcripts of all undergraduate and graduate study completed by applicant must be mailed directly by the college or university to the National Association of Junior Auxiliaries, Inc.
After filling out the application form, check form for errors and clarity. If you need to make changes or corrections, simply re-enter the corrected information.

After you have reviewed the completed application form you must print a hard copy, sign and date it. Send original and six (6) copies along with all enclosures to the address listed above.
